Overview
This is now the most popular cosmetic surgery procedure with women of all ages. The choice in style, shape and texture of implants available is wide, and techniques to ensure a faster more comfortable recovery have been improved such that the overall result now achieves a more natural looking breast.
The Implants
In Australia and New Zealand the most popular implants are silicone cohesive gel. The contents resemble a firm set jelly and the envelope is a silicone shell with either a textured or smooth surface.
There are several approved suppliers of these implants and each has a symmetrical (round) or anatomical (tear drop) shape option. The choice of implant needs to be discussed with the surgeon to ensure the correct base width and projection are recommended for the volume size required.
The Surgical Plan
The operation can be performed in a hospital or accredited day surgery centre with a full general anaesthetic or local anaesthetic as sedation.
Once the patient is asleep the surgeon makes an incision through the skin and then lifts the breast tissue and, if necessary the muscle, to make a pocket in which to place the implant. The incision is closed with stiches under the skin.
Incision Choices
The patient decides where the incision is to be placed. The options are:-
Under the natural fold of the breast; this is the most popular and is termed the
concealed incision
Around the edge of the nipple; this will result in an invisible incision called
‘invisicion’
In the
arm pit; this can leave a visible scar and is the least popular of all approaches.
The Operating Day
This is a day only procedure taking from 45 mins to 1.5 hrs of operating time- the entire process from arrival at the hospital to discharge takes ½ day. Drain tubes are rarely used in this procedure and the patient will have a small dressing over the wound site.
Each surgeon has a specific protocol for his patients and these will be discussed in detail during the initial consultation.
FAQs
How will I feel?
There will be some pain requiring medication for 3 to 5 days; swelling and discomfort are normal for up to 4 weeks. After this time the breasts will become softer and look and feel more natural.
Most patients find the shape of the breasts continues to improve over a 4 month period.
What are the risks?
The breasts can feel hard and look uneven if the scar tissue surrounding the implant contracts - if this happens there will be a short revision necessary
The wound can be slow to heal or breakdown if the patient places pressure on the site within the first 3 weeks, therefore all upper body work and exercise is not recommended during this period
The breast can look unnatural if the implant is positioned incorrectly.

Each implant supplier has their own guarantee and this will be given to you as part of the decision making process.

Breast Uplift and Reduction
Overview
Reduction mammaplasty is an aesthetic surgical procedure aimed at reducing overly large or lifting sagging breasts to a more comfortable shape and size.
This procedure is indicated if you feel uncomfortable with large or sagging breasts. There may be pain in the breasts, shoulders, lower neck or back or discomfort during physical activity. A breast operation minimises these problems, enhances the posture, and helps improve the overall appearance and self-esteem.
There are currently several techniques for reduction mammaplasty. They all aim to remove excess breast tissue, re-contour the breast, and reposition the nipples. Your surgeon will take your physical condition and requirements into consideration, and will determine which technique would be best for you.
Thousands of reduction mammaplasties are performed each year. The vast majority of women are very happy with their improvements and often say they wish they’d had the operation sooner!
The Surgical Plan
The operation normally takes between 2 to 3 hours and may be performed in a day stay hospital provided the patient has suitable aftercare, It is unusual for any post operative pain although swelling, bruising are common and may persist for 7 to 10 days.
Drain tubes are not usual for this procedure. Post operatively, the patient will wear a support bandage or garment for 3 to 5 days. Stitches are dissolving.
Re-shaping the breast by lifting the skin and removing excess tissue is now more frequently requested. The newer techniques offer minimal incisions with the end result showing less evidence of the operation.
Liposuction
Removal of fatty breast tissue to reduce size offers scar free surgery, however this technique generally does not give a lift and although the patient may reduce 1 to 2 cup sizes, the shaping and firming offered by alternate techniques, do not apply.
“Do-Nut” Lift
The less obvious scar involves an incision around the edge of the nipple to “drawstring” the breast giving a minimal lift. The nipple may be reduced and even repositioned during this procedure. Inserting a surgical mesh to support breast tissue internally has greatly improved the outcome of this surgery.
“Lolly Pop” Lift
A single incision, around the nipple and in a line to the fold of the breast, will minimise the incision and obvious scar and can give a lift or reduction in size to the breast.
The “T” Incision
For large reductions and significant lift, including repositioning the nipple, this is the one which gives the best result.

Will I have large scars?
The aesthetic outcome from these procedures is very dependant on your surgeons skills. Ensure you see his post operative results before proceeding. Check if the stitching is neat and dissolving.
Will I have pain?
Post operative pain is not usual with this operation
Can I still breast feed?
This operation may limit this possibility and many surgeons advise patients to wait until they have had children before proceeding.
How long will my result last?
This is very dependant on weight gain or loss, pregnancy and breast feeding. Given there is no dramatic change in the size of the breasts the results should be long lasting.
