Penis Enlargement - Phalloplasty

INTRODUCTION
Increasing numbers of men enquire about penis enlargement procedures to lengthen and thicken the penis. Although this procedure has been available for several decades, it is in the last 10 years that the procedure has become generally accepted as a bonafide operation of benefit to those patients who consider their penis to be too small. It has been explained by specialists internationally that whilst penis enlargement does not necessarily improve sexual function, however, the self image of the patient can be improved sufficiently, to eventually impact on sexual performance. Penis enlargement is an operation which should not be considered because of relationship pressures - the only benefit is to the patient themselves. As with all Cosmetic Surgery if the patient feels more confident as a result of surgery then it has been worthwhile.

PROMOTED TECHNIQUES
• Lengthening -
releasing the suspensory ligament which holds in a section of the penis behind the skin in the pelvic area.

• Widening - Increasing the circumference by grafting fat (taken from under the fold of the buttocks) under the skin of the shaft.

These procedures can be performed separately or together.

SURGICAL STATISTICS
Steps of the operation
Incisions are made in the natural crease of each side of the buttocks;
The fat is separated from the outer layer of skin;
The incision is closed with dissolving stitches;
An incision is placed at the base of the penis in the mid line of the scrotum. The suspensory ligament is released;
An incision is made either side of the circumcision line or if the patient is uncircumcised a circumcision is performed;
The fat graft is placed under the skin;
Both incisions are closed with dissolving stitches

The operation will take approximately 2 to 3 hours in hospital under general anaesthetic, however the patient may return home the same day if he wishes.

Pain relief may be requested for 7 to 10 days and most men require a minimum of 7 days off work. Stitches are dissolving and the surgeon will normally request a follow up consultation at 3 to 4 weeks post operatively.

MOST FREQUENTLY ASKED QUESTIONS

What is the average penis length?
The average differs according to race and height. However some research indicates that the average length for the European male is between 7 to 10cm in the flaccid state and 12cm to 18cm when erect.

What is the expected increase?
This varies with every patient and may be from 2.5cm - 6cm. The average increase with a sample of Clinical Beauty patients in 1999 - 2000 shows an average of 3cm. These measurements are taken in the flaccid state.

How much is the circumference increased?
The final increase depends on the thickness of the graft and the re-absorption of the fat over time. An average of Clinical Beauty patients in 1999 - 2000 post operatively indicates 2cm or approximately 1/3 of the pre operative girth.

Is the scar visible?
If the incision is taken at the base of the shaft, close to the scrotum the scar is not noticeable, providing the area heals well. The scar around the head of the penis is likened to a circumcision scar and settles in time. However, if the incision is taken on the top side of the shaft and in the pubic area the scar may be visible as the rick of wound break down and infection is higher.

WHAT CAN GO WRONG WITH PENIS ENLARGEMENT?
Specific risks must be discusses in detail with you surgeon. He will explain the steps he takes to avoid the complication and how he deals with each if they occur. These are: - wound breakdown, infection, bleeding, scarring, sensitivity, aesthetic evaluation.

Visible scarring is a concern and is a distinct possibility if the incision is made on the top side of the penile shaft.
